Processing Times
Routine         4-6 weeks

Expedited      2-3 weeks (Expedited service is an additional $60)

If you are traveling in less than 3 weeks, or need a foreign visa within 4 weeks, you may call the National Passport Information Center (NPIC) 1-877-487-2778 for an appointment in person.

You must apply in person using Form DS-11 if at least one of the following is true:

  • You are applying for your first U.S. Passport
  • You are under age 16
  • Your previous U.S. passport was issued when you were under age 16
  • Your previous U.S. passport was lost, stolen, or damaged
  • Your previous U.S. passport was issued more than 15 years ago

You will need to bring the following to your appointment:

  • Form DS-11 completed in black ink only
  • Proof of Citizenship such as
    • Certified U.S. birth certificate; or
    • Previous fully valid U.S. Passport; or
    • Report of Birth Abroad; or
    • Certification of Birth Abroad; or
    • Certificate of Citizenship or Naturalization
  • Proof of identity such as a driver’s license, plus a photocopy front and back on one-sided paper
  • Recent passport photo (taken within the last 6 months). Do not attach photo to application
  • Fees
    • Check or money order payable to the US Department of State
    • Cash, check, debit or credit payable to the City of Woodinville for execution fee

    • Is submitted with your application
    • Is undamaged (other than normal "wear and tear")
    • Was issued when you were age 16 or older
    • Was issued within the last 15 years
    • Was issued in your current name (or you can document your name change with an original or certified copy of your marriage certificate, divorce decree, or court order)

    Adult renewals are only processed at an acceptance facility if your recent passport has been damaged, lost, or stolen, or if it has been expired for more than five years.

    Form DS-11 completed in black ink only

    You must submit one of the following U.S. citizenship documents for your child. Your evidence must be an original or certified copy. Photocopies and notarized copies are not acceptable.

    • Fully-valid undamaged U.S. passport (may be expired)
    • U.S. Birth certificate that meets the following requirements:
      • Issued by the city, county, or state of birth
      • Lists applicants full name, date of birth, and place of birth
      • Lists the parent(s) full names
      • Has the date filed with registrar’s office (must be within one year of birth)
      • Has the registrar’s signature
      • Has the seal of the issuing authority
    • Consular Report of Birth Abroad or Certification of Birth
    • Certificate of Citizenship

    Show Parental Relationship: you must submit documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport. The following may be used to show parental relationship:

    • U.S. birth certificate (also evidence of U.S. citizenship)
    • Consular Report of Birth Abroad (also evidence of U.S. citizenship)
    • Foreign birth certificate
    • Adoption decree
    • Divorce/custody decree

    Please note: Some documents, like a U.S. birth certificate, show both U.S. citizenship and parental relationship. These documents must be originals or certified copies (not photocopies).

    • Parents Proof of identity such as a driver’s license, plus a photocopy front and back on one-sided paper
    • Recent passport photo (taken within the last 6 months). Do not attach photo to application
    • Fees – two separate payments will be required
      • Check or money order payable to the US Department of State
      • Cash, check, debit or credit payable to the City of Woodinville for execution fee

    Things to remember:

    • Minor (applicant) is required to be present for appointment
    • All questions on the application are about the person (minor) applying
    • Minors should use someone other than a parent as an emergency contact
    • Social security number is required on all applications
    • Mother’s maiden name is required under ‘parents name at birth’

    Applicants Age 16 and 17

    Applicants age 16-17 must apply in person using Form DS-11.

    You will need to bring the following to your appointment:

  • Form DS-11 completed in black ink only
  • Proof of Citizenship such as

    • Previous fully valid U.S. Passport; or
    • Report of Birth Abroad; or

  • Recent passport photo (taken within the last 6 months). Do not attach photo to application

  • Parental Awareness
    • One parent is required for applicants who are 16 or 17 years of age (see Parental Awareness below)
  • Check or money order payable to the US Department of State
  • Cash, check, debit or credit payable to the City of Woodinville for execution fee

Parental Awareness

If you have your own in-state identification (in-state, fully valid driver’s license, not temporary or learner’s permit) you may apply for a passport by yourself. However, we strongly recommend that at least one parent accompany you to show parental awareness.

Examples of Parental Awareness:

  • A parent appearing with you in person when you apply for your passport
  • A signed statement consenting to issuance of a passport from at least one parent (should be accompanied by a photocopy of that parent’s ID)

If you do not have a fully valid driver’s license (not temporary or learner’s permit), you must show at least one parent knows that you are applying for a passport.
